By my signature below, I promise that the information in this employment application (and accompanying resumé or documentation, if any) is true and complete, and I understand that any false or misleading information or significant omissions may disqualify me from further consideration for employment and may lead to my dismissal from employment if discovered at a later date. I agree to immediately notify Michigan Shores if I should be convicted of a felony, or any crime involving dishonesty, breach of trust, controlled substances, sexual misconduct, abuse or violence, while my job application is pending, or during my period of employment, if hired.
I understand that this application does not create a contract of employment. I understand that, if hired, I am obliged to comply with any and all current and subsequently adopted Michigan Shores policies, including but not limited to the employee handbook. I understand and
agree that, if hired, my employment is for no definite period of time, and may, regardless of the date of payment of my wages or salary, be terminated at any time for any reason, with or without notice. I understand that no person is authorized to change any of the terms mentioned in this employment application.
I understand that if applying for any position for which consumer reports can be considered under applicable law a consumer report, which may contain information regarding my credit worthiness, credit standing, credit capacity, character, general reputation, personal characteristics, criminal history, or mode of living, may be requested from Ferret Background Check and used in whole or in part for the purpose of serving as a factor in regard to my eligibility for employment, promotion, reassignment, or retention as an employee.
I further understand that an investigative consumer report, which may include information as to my character, general reputation, personal characteristics, or mode of living, may be made. Such a report may result from interviews of others, concerning such matters as my prior employment, capabilities, and qualifications, or concerning employment problems, should any arise, such as sexual harassment, workplace violence, fraud or theft. I understand that I may request further information from the company regarding the nature and/or scope of any investigation. Such disclosure will be made to me within five days of the date on which the company receives my request or the date the report was requested, whichever is later.
I also acknowledge receipt of a copy of “A Summary of Your Rights Under the Fair Credit Reporting Act.” By my signature below, I hereby authorize a consumer report and/or an investigative consumer report to be obtained.
